In return for the Animal Rescue League allowing me to adopt the above-indicated animal, I hereby

make the following presentations and promises to the League:



1. I am 18 years of age or older and have carefully read the terms and conditions of this agreement.



2. I understand that:

a. The League may exchange the animal for another if the animal is not satisfactory to me, but that

this privilege must be exercised within ten (10) days of the adoption and the exchange selection

made within 30 days thereafter;

b. I may exercise this exchange privilege only once; and

c. During the initial 14 days after adoption a VCA Animal Hospital will provide follow-up medical

care for certain limited conditions as outlined in the adoption packet.



3. If I return the animal to the League for any reason, or it should die while in my possession, I

understand and agree that I will not be entitled to reimbursement of any fees, including adoption

fees, and other purchases or payments made by me to the League. I will at no time assert

any claim, charge or demand of any kind or nature against the League for any charges that have

been incurred by me, including veterinarian fees, in connection with the animal.



4. If a family member develops allergies and presents a doctor's certificate within seven (7)

days of adoption, an exchange may be granted.



5. The animals available for adoption came here from a variety of sources. All animals are

examined upon entry, and their health is routinely monitored while at the shelter. The veterinarians

at the League have made a substantial effort to see that the adopted pets are in good health.

There is always a chance that an animal is incubating a disease without showing any clinical

signs. All new adopters should contact and follow the advice of the VCA Veterinarian as soon

as possible after adoption.



I agree that after the 14th day, the animal's medical care is my sole responsibility.



6. I will provide proper and sufficient food, water, shelter and kind treatment for the animal at all

times. I will also have the animal immunized and examined annually by a veterinarian at my

expense. This veterinary care will include heartworm prevention, flea prevention, rabies

vaccinations and licensing and other preventative vaccinations.



7. I will keep the animal isolated from other animals in my household for the first ten days after

adoption. Introducing a new animal to an existing animal at home is a gradual process, which

requires time for both animals to adjust to the new environment. An isolation period is key

for this adjustment. Also, if either animal is incubating any illness, ten days will be enough

time for presentation of the illness, and help prevent the existing animal from becoming sick.



8. The League provides no guarantee of any pet's age, behavior, breed, general disposition

or aggressive or vicious tendencies.







9. I understand that this pet may chew, eat plants, scratch furniture and may not be housebroken.

I also understand that this pet will require at least two to six weeks to adjust to my home.



10. I understand that no animal shall be adopted as an outside only animal and no dog shall be

adopted as a guard dog.



11. I understand that no animal will be adopted to a person who is trying to obtain the animal for

another person who has previously been refused, or as a gift for someone else.



12. I understand no animal will be adopted to a person who displays any unwillingness to

comply with League policies, or whose attitude is indifferent or contemptuous toward the

adoption process.

I accept the animals as is, including all known or unknown conditions, and assume all risk of ownership, including the risk of injury or damage caused by the animal to myself or others, such as animal bites, and assume liability for costs of all necessary medical treatment for the animal. On behalf of myself, my heirs, personal representatives and assigns, I hereby release, discharge, indemnify and hold harmless the League and its directors, officers, employees, and agents from any claims, causes of actions and demands of any nature, whether known or unknown, arising out of or in connection with my adoption, care and ownership of the animal. ________ Initial



This agreement may be specifically enforceable by the League through judicial proceedings. In the

event I breach this agreement and the League files suit to enforce its terms or to defend any claims

hereunder, I will pay any court costs and attorney's fees incurred by the League in connection

therewith.



I agree to provide my pet with proper veterinary care for the rest of his/her life. I understand that the

League takes reasonable precautions to provide healthy animals for adoption, but it is unable to test

for every possible disease.

The Atlanta Humane Society and Society For Prevention Of Cruelty To Animals, Inc. (AHS) cannot guarantee the health or temperment of any animal released from the shelter. We believe this animal to be in good physical condition. However, we suggest you take the animal to a veterinarian as soon as possible for a thorough check-up.

I hereby adopt this animal in its present condition and personally agree to care for it in the most humane manner. I promise not to use this animal for vivisection or in any inhumane way. I will provide adequate care and observe all city, county and state animal regulations.

ACKNOWLEDGEMENT, RELEASE, WAIVER AND DISCLOSURE

I hereby accept possession, title, and responsibility of and for the animal described above, and in consideration hereof, and at my own risk, and hereby covenent and agree not to sue or assert any claim, demand or action against or with respect to the Atlanta Humane Society for myself and others directly or indirectly regarding or arising from the animal and I hereby hold harmless, and agree to indemnify and defend the Atlanta Humane Society for any rights and claims against the Atlanta Humane Society which I or others now or in the future have or may have for any damages or injuries to person or property caused by or with respect to said animal, and for myself and others release and waive any such rights and claims. I understand and agree that I have the right to do this and that this is furnished in favor of the Atlanta Humane Society, here meaning and including the Atlanta Humane Society and Society For Prevention Of Cruelty To Animals, Inc., Atlanta, Georgia, and related Shelter, Auxiliary, and Clinic, and their officers, directors, members, volunteers, employees, agents, respectively.

In addition, I acknowledge that I am 18 years or older and have read, understand and specifically agree to these provisions and agree to be bound by and to comply with the terms and conditions set forth herein.
